Risk, Consent to Participation, and Waiver: 

While Ontario Streams will make every effort to minimize exposure to known risks associated with and/or the participants minor’s participation in the the Self-Guided Nature Walk Program and physical activities that for part of the Program involve risk to you and/or the participant minor and participating parents and guardians. These risks include: 

• Irregular, uneven or slippery terrain
• Becoming disoriented or lost
• Falling objects
• Changes in weather conditions, including extreme heat or extreme cold
• Health risks from poor air quality
• Branches and springing vegetation 
• Encounters with natural features such as plants, insects, and animals
• Encounters with water hazards such as ponds, rivers, lakes, and streams and associated hazards such as flooding

Liability Waiver and Photo Release:

Terms and Conditions  

• Participants agree to release Ontario Streams from all liability, whether direct or indirect, and waive all claims, demands, damages, costs, expenses, actions or causes of actions arising out of or in consequence of any death, injury, loss or damage to my person or property or that of my child or ward, however caused, while attending at or participating in the activities; including but not limited to, breach of the Occupier’s Liability Act
• Participants agree to further waive and release Ontario Streams from an recourse which I may now or hereafter have resulting from any decision of Ontario Streams
• Participants agree to indemnify Ontario Streams from and assume full responsibility for any claims demands, damages, costs, expenses, actions or causes of actions it may incur arising out of or in consequence of any death, injury, loss or damage due to my participation or that of my child’s or ward’s attendance or participation in the activities  
• Participants have the right to refuse any activity they believe is unsafe  
• Participants are responsible for all injuries or incidents, no matter how minor, to an Ontario Streams staff member  
• Participants are responsible for any allergies or health conditions which may affect them or limit their involvement to an Ontario Streams staff member prior to beginning the event.
• Participants consent (if unable to provide it or deny it myself) as a participant in the event of injury or illness participants will seek further medical attention and/or first aid that potentially may be required to the extent determined by, and at the discretion of Ontario Streams staff, emergency medical services, and licensed medical professionals.    

Photographic Release  

• I agree to grant permission for the use of images of myself captured during Ontario Streams activities; these images will be used solely for the purposes of Ontario Streams and their funders/partners promotional material and publications, and I hereby waive any rights of compensation or ownership.
